Downloadable Track Forms
The middle 5 forms are
required by the school. If you completed a Physical Examination Form
since the Spring of 2007 and it is on file in the Athletic Office, or you are a
freshmen, you do not
need to fill that form out again. The other 4 forms are required to be
completed for each new
season.
The Track and Field Policy needs to be read, and
the last page needs to be detached, signed
by you and one parent, and returned to the coaching staff. Keep the rest of
the Policy for future
reference.
The 2009 Track Fees form must be filled out, a check
made out the St. Francis Track, and
returned to the coaching staff no later than February 14.
All forms must be returned to
the coaching staff no later than Wednesday, February 28 or no
uniforms will be issued and you will not be allowed to participate in Track and
Field this year.
